Supporting transitions

This useful document from Moray Council has as its basis the framework for planning transitions as outlined in the Education (Additional Support for Learning) (Scotland) Act 2004. Following the Code of Practice and related regulations, it sets out the authority’s approach to supporting transitions in the following areas:

• into pre-school provision
• from pre-school provision to primary
• from primary to secondary
• from secondary to post-school

It includes sample agendas for each transition meeting. This is a working document that will be revised in the light of feedback and any comments would be welcomed.
